Format for applying for Grant funds

Requests for funds should be made in writing, which must include:

• Pertinent information about the organization and its activites.
• Organization budget.
• Copy of the organizations taxes exempt status along with your most recent Form 990, a list of other donors and their level of support.
• Description of project for which grant is being requested.
• Project objectives and the means by which you'll meet those objectives.
• The age and number of children benefiting from the program.
• Project time span.
• Project budget.
• A description of current or matched funds for the program and plans for the future funding.
